This page supports our responsibilities under the Serious Incident Response Scheme (SIRS) and reflects our belief that every voice matters.
Enhanced protections under the new Aged Care Act 2025 ensure your identity is protected and you cannot be punished for speaking up.
Anyone connected with Hosanna’s aged care services can submit a report:
– Aged care clients
– Family members or carers
– Support workers or staff
– Advocates or visitors
– Community members
– Anonymous reporters (identity fully protected)
You can use this form to report incidents such as: – Abuse (physical, sexual, emotional, financial) – Neglect or poor care – Unexplained injury or sudden deterioration – Unexpected death – Inappropriate use of force or restraint – Unlawful sexual contact or inappropriate sexual conduct – Any behaviour that causes concern or risk – Breaches of consumer rights under the Statement of Rights
If you’re not sure whether something should be reported — please submit the form anyway. We will review every report seriously.
Whistleblower Protections: Under the strengthened Aged Care Act 2025, you have enhanced legal protections when reporting serious concerns. This means:
– Your identity will be protected (with limited exceptions for investigations)
– You cannot be punished, threatened, or treated unfairly for making a report
– You can make reports anonymously
– You have the right to seek compensation if you suffer detriment for reporting
Once you submit the form:
– Our Quality and Compliance Team will review your report immediately.
– Priority 1 incidents (serious risk like abuse, death, unlawful sexual contact): reported to the Aged Care Commission within 24 hours.
– Priority 2 incidents: reported to the Aged Care Commission within 30 days. – If you provide your contact details, we will follow up with you as soon as possible.
– Dual reporting: If you also receive NDIS services, serious incidents will be reported to both the Aged Care Commission and NDIS Commission.
– All reports are handled confidentially and with care.
