What is the Support at Home Program?

The Support at Home program is the Australian Government’s new aged care initiative designed to help older Australians remain living independently at home for longer.

This program replaced the Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs on 1 November 2025, offering a simpler, fairer and more flexible approach to in-home care.

With eight new classification levels (up from four), Support at Home provides more precise funding matched to your individual care needs, ensuring you receive the right level of support at the right time.

Additional Charges
Travel: $1.50 per kilometre (return trip)
Minimum Visit: 30 minutes, then 15-minute increments
Cancellation: Full fee if cancelled within 24 hours
After-Hours: Saturday +15% | Sunday/Public Holidays +25%

Standard hours: Monday-Friday, 6:00am-8:00pm. All prices effective from 1 November 2025.

Transitioning from a Home Care Package

Transparent pricing for all Support at Home services. Your contribution depends on your pension status and the service category.

The "No Worse Off" Principle

If you were assessed or approved for a Home Care Package on or before 12 September 2024, you are protected under the “no worse off” principle. This means you automatically transitioned to Support at Home without needing to reapply or undergo a new assessment.

Same or Lower Costs

If you currently pay no fees, you won’t be charged under Support at Home. If you pay fees, you’ll pay the same or less.

Funds Carry Over

Your existing package level and unspent funds carry over, allowing you to continue using them for eligible services.

Lifetime Cap Protected

The lifetime cap of $130,000 (indexed) on your contributions remains in place.

Your contribution towards services is based on an assessment of your income and assets. Different contribution rates apply depending on your pension status.

Rules for Unspent Funds

Existing Home Care Package participants retain all unspent funds. They carry over in full and remain available for use under Support at Home.

You can use your HCP balance to:

  • Pay for approved services once your quarterly budget is exhausted
  • Purchase assistive technology or home modifications (AT-HM) to meet assessed needs

Quarterly Carryover: Under Support at Home, you can carry over up to $1,000 or 10% of your quarterly budget (whichever is greater) into the next quarter.

Short-term Pathways

In addition to ongoing classifications, Support at Home offers three short-term pathways for specific needs.

Assistive Technology & Home Modifications

Provides upfront funding to access equipment and home modifications that make daily life safer and more accessible, without needing to save from your ongoing budget.

Includes: Mobility aids, handrails, ramps, shower modifications, and approved equipment
Restorative Care Pathway

Short-term intensive allied health and nursing care designed to help you regain independence after illness or injury and prevent the need for higher ongoing care.

Duration: Up to 16 weeks
Funding: $6,000 (or up to $12,000 if eligible)
Assistive Technology & Home Modifications

Provides upfront funding to access equipment and home modifications that make daily life safer and more accessible, without needing to save from your ongoing budget.

Eligibility: 3 months or less to live
Duration: 12 weeks of additional support

Frequently Asked Questions

Support at Home is the Australian Government's new aged care program that replaced Home Care Packages from 1 November 2025. It provides government-subsidised services to help older Australians live safely and independently at home for longer.
To access Support at Home, you need to contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to register and arrange an aged care assessment. Once assessed and approved, you can choose Hosanna Health Care as your provider.
You may be eligible if you are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), need help to continue living at home, and are assessed as requiring support through an aged care assessment.
Wait times depend on your priority category (urgent, high, medium or standard) and when you were approved. Once you choose Hosanna Health Care as your provider, our intake team will visit you within 48 hours to begin your care.
Support at Home funds a wide range of services including clinical care (nursing, allied health), independence support (personal care, transport, social support), everyday living (cleaning, meals, gardening), assistive technology and home modifications.
Yes, you can use your Support at Home budget for approved assistive technology and home modifications. There is also a separate AT-HM Scheme that provides upfront funding for these items without using your ongoing services budget.
 
Your contribution depends on your income and assets assessment. Clinical services have no contribution, while independence and everyday living services have varying contributions based on your pension status. Full pensioners pay 0-17.5%, while self-funded retirees pay up to 50-80%.
